The following are loan documents that may be needed for the completion of a loan:
First Time Homebuyer - Seller Affidavit - OHFA Form
The following document is only required by Seller(s) in the event a borrower has selected the State of Ohio First Time Homebuyer Program, aka the OHFA (Ohio Housing Finance Agency) First Time Hombuyer Program, and is to be Completed by the Seller(s), Signed before a Notary, and Sent back to My Attention at First Place Bank. It is important that we receive this form with enough lead-time so that we may get it to the State for their Review of including, but not necessarily limited to, the borrower's forms and income.

Authorization to Release Information - First Place Bank
The following document is a requirement at Loan Application or at the Pre-Approval stage. In short, it is our "permission slip" to allow us to pull credit, verify employment, deposits, or rent among other things that are listed on the form itself. By signing it and faxing it back to us, you are allowing us to do the things necessary to assist you in obtaining financing.

Insurance Information & Mortgagee Clause
Once a borrower has applied, it is important for that borrower to set up a policy to cover a 1-Year Period covering the upcoming year and to obtain either a "paid receipt" (a receipt reflecting payment to the borrower's insurance company) or an "invoice" to be paid through closing along with "evidence of insurance" to be sent/faxed/e-mailed over to us by the agent/insurance company.
